The mRNA boundaries are calculated using the gene's default feature set from NCBI, mapped onto the UCSC Golden Path. PharmGKB sets gene boundaries by expanding the mRNA boundaries by no less than 10,000 bases upstream (5') and 3,000 bases downstream (3') to allow for potential regulatory regions.

CYP2B6 is a member of the cytochrome P450 family of important pharmacogenes and makes up approximately 2-10% of total hepatic CYP content [Article:18781911]. CYP2B6 is also expressed in brain, which may be an important factor in the metabolism of CNS-acting drugs and neurological side-effects of drug treatments [Article:15237857]. CYP2B6 is responsible for the metabolism of 4% of the top 200 drugs [Article:18695978]. CYP2B6 is highly inducible by several drugs and other xenobiotics [Article:9280407]. There is high inter-individual variation in CYP2B6 expression, estimated at 20-250 fold, which may be attributed to differential transcriptional regulation as well as inherited genetic variation [Article:9280407, 11470993, 14551287]. A large number of variants have been reported ( see the CYP allele nomenclature committee website ), including many that are present at high frequencies and several which show linkage resulting in multiple haplotypes. These haplotypes are often observed at highly different frequencies in different racial and ethnic groups (reviewed in [Article:17638512]). CYP2B6 has been shown to undergo significant splicing [Article:14551287]. Although multiple splice variants have been reported, functional relevance of most of these needs further investigation. One of the splice variants (SV1) has been shown to be linked to an important haplotype [Article:14551287, 18171905]. Like other CYPs, CYP2B6 has a high level of homology with a pseudogene, in this case CYP2B7P1, which can interfere with genotyping if assays are not specific enough [Article:18695978].

Transcription of CYP2B6 is regulated by the nuclear receptors PXR and CAR, coded for by the genes NR1I2 and NR1I3 respectively. They activate transcription by binding at the proximal response element (PREM at -1.7km upstream of the transcription start site) and distal response element (XREM at -8.5kb) in the CYP2B6 promoter [Article:11502872, 12571232]. There is thought to be cross-regulation between CYP2B6 and CYP3A4 and UGT1A1 and hepatic drug transporters that are also regulated by these transcription factors [Article:18781911]. The impact of variants of CYP2B6 on regulation has not been extensively studied. Many of the CYP2B6 substrates such as cyclophosphamide, phenobarbitol, rifampicin, phenytoin, artimesin, carbamazepine, efavirenz and nevirapine can also induce its expression through PXR and CAR [Article:17638512]. (See below for a larger list of substrates and inhibitors of CYP2B6 protein.) Although gender has also been shown to influence CYP2B6 expression there have been conflicting reports in the literature; this conflict might be due to other confounding factors such as race and environmental factors, discussed in [Article:18695978].

As CYP2B6 is the major enzyme involved in the metabolism of efavirenz and nevirapine, its pharmacogenomics has become relevant for the treatment of HIV. Efavirenz has a narrow therapeutic window with severe CNS side effects associated with high plasma concentrations and treatment failure associated with low concentrations. Variants CYP2B6:516G>T and CYP2B6:983T>C as well as haplotypes CYP2B6*4 and CYP2B6*6 have been associated with adverse effects of efavirenz treatment. Clinical significance of CYP2B6 variants has also been implicated in cyclophosphamide therapy and smoking cessation in response to bupropion. Although several important variants and haplotypes have been identified, the drug-variant interactions for CYP2B6 still need further investigation.

Splice Variant Summaries

CYP2B6:SV1

There have been 5 splice variants reported for CYP2B6 (see Lamba et al., 2003, [Article:14551287]). The SV1 splice variant lacks exons 4, 5 and 6 and is approximately 500bp long (the full cDNA is approximately 1500bp long) [Article:14551287]. Association of the CYP2B6:516G>T with the occurrence of CYP2B6:SV1 was first suggested by Lamba et al. [Article:14551287] and has been more recently validated by Hoffman et al. [Article:18171905]. Association of the CYP2B6*6 haplotype, which includes the CYP2B6:516G>T variant, has been linked to severely decreased CYP2B6 expression and function [Article:18171905].
